The Concept

The concept behind Xybion Health’s Emidence platform was to bring a complete evidence based  and outcome focused digital platform to the Workers’ Comp marketplace that was capable of being deployed across all stakeholder including providers, payors, employers and patients.  This unique model was built from the ground up on a web-based platform that allows for “any device” access to the platform.

There are three core components to the Xybion Health solutions

More than a fee scheduler

The configuration of the bill review system is a consultative process. Our experience is that many clients have a broad range of claims philosophies and that while state legislatures and state regulators make rules, occasionally the state courts differ when litigation occurs. Many aspects of medical bill adjudication are option driven with almost infinite possibilities for custom configuration.

  • Appropriateness of care – That is, does the treatment or service match the diagnosis? Also, the system reviews for whether a particular type of provider is allowed to invoice for the service billed. Example – A Registered Physical Therapist may not bill for a physician office visit.
  • Unbundling – On occasion, a provider will invoice separately for a service or supply that was billed under a “global” code. Since the supply or service was already allowed under a previous billing the “unbundled” code should be denied.
  • Duplicate billing – We will identify and deny payment for services or supplies that duplicate previous charges.
    Utilization Review – While payment may not be denied based solely upon over utilization, a warning may appear on the EOB when treatment exceeds that which other patients with the same injury have received. This warning alerts both the provider and the adjuster to potential over treatment and requests objective documentation from the provider.
  • Level of Service – Many times providers will submit billing for a level of service that does not correlate to the severity of the injury. For example when the diagnosis of sprains and strains of the neck (ICD-9 847.0) is submitted along with a high level of service (CPT 99215) the service would be reviewed as 99214 or 99213. The service 99215 requires a diagnosis that carries a high level of medical decision-making and a high risk of death or permanent disability and is therefore inappropriate.

Bill Review Software – Bill Intake

Xybion’s Emidence provides a comprehensive bill review platform as part of its overall solution. Within the Bill review platform is a comprehensive bill intake module designed to streamline the overall intake process. Our interface provides a bundle of options for billing that can save time and effort for the patients and the providers.

  • EDI : Data is first transferred via EDI which then gets loaded onto the CORE system for processing with the desired state and review settings.Some of them  directly return while others enter the adjuster queue for review.
  • Scanned Bills: Our platform also allows clients to load bills at their location and images get loaded to the FTP site. The bills are then conveyed to the CORE system for processing with the desired state and review settings.
  • Email/Mail: Clients can also mail their bills,which are scanned on site and keyed into the CORE system for processing. Original bills are either returned or shredded. Reviewed bills are printed and returned to the patient.
  • Manual: On site manual uploading to the CORE system can also be done.
  • Fax: These also have to be manually uploaded to the CORE system for processing.
  • FTP: Bills can also be transferred via File Transfer Protocol(FTP) which is a standard network protocol for relaying data between a client and server on a computer network.

The billing review software platform includes a robust, real-time dashboard that displays KPI’s and other key data sets  that are role based  and  integrate number of bills by client,daily savings,status of multiple bills,workload and Assignee s information. It also evaluates if a particular provider is eligible to invoice for the service billed, and if the treatment or service matches the diagnosis.

Diagnosis and CPT entry

It has all possible diagnosis options included that can be readily chosen and be auto-populated minimizing data entry. Along with this,it also has embedded rules that allow customizable triggers. For instance, when diagnosis codes do not support the CPT codes or services rendered, manual entry can be done.

EDI Support

Integration with PPO networks is an other added feature that evaluates pre existing relationships with multiple PPO providers to assist with additional savings. Bills that are uploaded to the CORE system bridge directly to the network to include the contract rates applied and thereby save money for the patients.

Fee Schedule
This platform is more than a fee schedule system. Along with doing the fee scheduling function accurately according to the National guidelines, it can also put a check to duplicate billing and over utilization. These guidelines are regularly updated and monitored by our expert team.

KFI : Key from Image

An other added unique feature is that it can auto populate all the details of the patients and providers from the uploaded health insurance forms. It is therefore aptly called as “Key from Image” as all the information is directly and automatically sought from the insurance form images.

Front Office: It can make automated patient appointments , send reminders and also verify if the patient is eligible for insurance.

Patient’s consultation: On the day of visit,our EHR platform can facilitate documenting all the medical history and exams done on the patient in detail. It has 500+ pre-built medical exams. It also documents the laboratory and other investigations done on the patient and auto generates SOAP notes and patient s summary. Also verifies if appropriate treatment is given for the respective patient s condition and prevents repeated entries and duplicate billing.

Billing Services: Integrated EHR with billing services is the first of its kind unique feature that can avoid improper billing and unnecessary delays. Insurance forms are auto filled with all the details of the patient and provider and is then sent to Insurance company for processing. Our platform is also compliant with various state and federal health laws.

California Workers Compensation

Senate Bill 863 passed on August 1, 2012, and was signed into law on Sept. 18, 2012. The bill makes wide-ranging changes to California’s workers’ compensation system, including increased benefits to injured workers and cost-saving efficiencies. The bill took effect on Jan. 1, 2013, although some of its provisions take effect at a later date.

Offered as a modular or complete solution, the platform provides software that manages all of the traditional business processes such as bill review, fee schedule, claims management, UR and case management, while bringing a fully functioning, evidence-based EHR for providers that traditionally manage a high volume of workers’ comp cases, such as Physical Therapy, musculoskeletal Wellness and Chiropractic practices.

The US has used ICD-10-CM since October 1, 2015.[ This national variant of ICD-10 was provided by the Centers for Medicare and Medicaid Services (CMS) and the National Center for Health Statistics (NCHS), and the use of ICD-10-CM codes are now mandated for all inpatient medical reporting requirements. There are over 70,000 ICD-10-CM codes, which is up from around 14,000 ICD-9-CM codes.

The use of ICD-10 for coding of death certificates and mortality data was mandated in the United States beginning in 1999.  The deadline for the United States to begin using Clinical Modification ICD-10-CM for diagnosis coding and Procedure Coding System ICD-10-PCS for inpatient hospital procedure coding was set at October 1, 2015, which is a year later than a previous 2014 deadline.  Before that 2014 deadline, the previous deadline has been a year before that on October 1, 2013.   All HIPAA “covered entities” must make the change; a pre-requisite to ICD-10-CM is the adoption of EDI Version 5010 by January 1, 2012.  Enforcement of 5010 transition by the Centers for Medicare & Medicaid Services (CMS), however, was postponed by CMS until March 31, 2012, with the federal agency citing numerous factors, including slow software upgrades.  The implementation of ICD-10-CM has been subject to previous delays. In January 2009, the date was pushed back by two years, to October 1, 2013 rather than an earlier proposal of October 1, 2011.

The configuration of the bill review system is a consultative process. Our experience is that many clients have a broad range of claims philosophies and that while state legislatures and state regulators make rules, occasionally the state courts differ when litigation occurs. Many aspects of medical bill adjudication are option driven with almost infinite possibilities for custom configuration.

  • Appropriateness of care – That is, does the treatment or service match the diagnosis? Also, the system reviews for whether a particular type of provider is allowed to invoice for the service billed. Example – A Registered Physical Therapist may not bill for a physician office visit.
  • Unbundling – On occasion, a provider will invoice separately for a service or supply that was billed under a “global” code. Since the supply or service was already allowed under a previous billing the “unbundled” code should be denied.
  • Duplicate billing – We will identify and deny payment for services or supplies that duplicate previous charges.
    Utilization Review – While payment may not be denied based solely upon over utilization, a warning may appear on the EOB when treatment exceeds that which other patients with the same injury have received. This warning alerts both the provider and the adjuster to potential over treatment and requests objective documentation from the provider.
  • Level of Service – Many times providers will submit billing for a level of service that does not correlate to the severity of the injury. For example when the diagnosis of sprains and strains of the neck (ICD-9 847.0) is submitted along with a high level of service (CPT 99215) the service would be reviewed as 99214 or 99213. The service 99215 requires a diagnosis that carries a high level of medical decision-making and a high risk of death or permanent disability and is therefore inappropriate.

It’s Time to Rethink How We Manage Musculoskeletal Injuries

According to the U. S. Bone and Joint Initiative, 126.6 million adults in this country (that’s 1 in 2) are affected by musculoskeletal injuries. The same source estimates that medical treatment and lost wages resulting from these conditions is more than $874 billion.

Alarming, well that’s only part of the equation. Just imagine what the total number equates to when adding lost productivity – especially with current opioid dosing practices taking its toll.

Challenges

While across the board we as claim, leave, care management and healthcare delivery professionals have yet to figure out the right combination to manage soft tissue injuries on a consistent and wide-scale basis. We can probably all agree that at a minimum, any dynamic and effective solution(s) must include:

  • Evidence-based Medicine/treatment delivery
  • Enhanced methodologies to cope with psychosocial issues
  • Accurate and reliable disability/impairment evaluation

Evidence-based Medicine

Though the term Evidence-Based Medicine is commonly used, explanations of the term employed by participants along the care and claims management continuum are anything but common. For this discussion, let’s agree to use the following widely accepted definition which is: The judicious use of the best current available scientific research in making decisions about the care of patients. Evidence-based medicine (EBM) is intended to integrate clinical expertise with the research evidence and patient values.

ACOEM, ODG, Medical Disability Advisor (MDA), MEDLINE and a host of other nationally recognized guidelines and search tools are readily available and have been put to good use, but uniform application, especially without automated integration, is not yet the norm.

Psychosocial Issues

While discussed often, how effective are practices employed to manage anxiety, depression, catastrophizing, and fear avoidance – which can be detrimental to recovery. Given that no two patients are alike, and study after study confirms the importance of managing patient characteristics and preferences regarding types of medical interventions, a holistic approach dealing with individual thought processes is no longer just a nice to have.

Consistent and Accurate Disability/Impairment Evaluations

If suffering from a chronic soft tissue injury or becoming permanently impaired isn’t hard enough, as an industry, our inability to apply findings, rules and benefits in an equitable and persistent manner only creates more problems. Take the following real life examples:

  1. During a discussion with a long-term risk manager from a very large firm, he pointed out what disturbs him the most is when two long-term employees, roughly the same age, doing the same job and suffering from the same injury get two drastically different impairment ratings. One gets a low rating, the other gets a rating exponentially higher. By the way, co-morbid conditions did not play a role. His beef was simple, did one get cheated and the other win the lottery, or was the high rating correct and the employee with the low rating treated unfairly. Isn’t accuracy and consistency what we are all looking for?

  1. At a major conference during a sit-down with a group of judges who adjudicate both workers’ compensation and disability benefits, the group pointed out that their ability to do a fair and impartial job relied heavily upon the information that they were provided. First, if the report is light and/or uninformative, how can their determinations be accurate? Garbage in, Garbage out. Next, given the time lag between when the report was written and when the proceedings occur, is what they are reviewing really an accurate depiction of the individual’s current state. Did they get better, or did they get worse? Either way it’s a real problem.

Though this all sounds bleak, it’s not. Recent technological advances in the assessment, treatment and evaluation of Musculoskeletal Injuries are readily available, and easily integrated into existing platforms. The end-result is consistent, accurate reproducible assessments enabling enhanced outcomes to become the norm.
